You need to file a police report about the cat and call your lawyer to get his visitation revoked.

His visits aren’t supervised if they’re in your home and there’s no one there to supervise him, obviously if he was able to kill your cat over two days.
Also sitting on the couch next to your violent ex sounds like you’re not really independent of him.
Edited to add: if he committed a crime in the presence of your children then it could be child endangerment. He’s gonna traumatize them and you might even have one that ends up behaving like their father. You need a lawyer and to put your kids safety first.

Ask your lawyer to help with the RO paperwork. Ask for the RO to have a provision for visitation to be switched to only at a visitation center, away from home, and to be supervised by a 3rd party professional. You’re lawyer should be able to name the type of acceptable professional- social worker or whatever. Include your ex pays all costs of the supervision, and your travel costs, loss of wage costs from taking time off work, etc. as a RO stipulation.
The police report will result in criminal charges or not, but you’ll need it as documentation for the RO and future custody hearing. Getyour lawyers advice what to attach but minimum should be witness statements from kids and vet report and vet bill.
