How many notes could you possibly need to take during a phone screen?
I think perhaps you need to learn some self-editing skills. I can’t imagine sending over such a bulky summary. Generally, when I submit candidates, it’s a very succinct 3-4 sentences. Beyond that, their resume should speak for itself.
You don’t need to write down everything. You also don’t need to organize your notes beyond figuring out how to state, basically but effectively, why the candidate is a solid fit. Pick the most major accomplishment, not ten of them. Just take notes, then look at them and edit down to what you actually need.

You need to work on your note-taking ability. You don’t need an entire page, grab the highlights that relate to the job, let the resume say the rest.
